Jeremy Calame PT, DPT

Dr. Jeremy Calame is the founder of MOOSE Physical Therapy, PLLC. From infancy his grandfather, John Sr., called him a “moose,” and is the inspiration behind the clinic name. He has been practicing physical therapy since 2010, but the pursuit of his career began in the 6th grade. As long as he can remember, he has always had a passion to help people and enjoys science, medicine and sports which is a perfect combination for his occupation. He knew that physical therapy was his calling and loves being an integral part in making a positive impact on his patient’s functionality. His Christian faith has molded his mission to serve his patients with genuine care. He enjoys staying updated on current research and interventions to aide his patient’s recovery through a caring, compassionate, and empathetic delivery. Jeremy believes in the art of physical healing through a hands-on approach and incorporates his manual therapy skills into his patient’s rehabilitation. As a therapist, he treats outpatient orthopedic and sports medicine patients, as well as a specialty in men’s health; hence the acronym Men’s health Outpatient Orthopedic & Sportsmed Exercise, MOOSE.
He began his career training alongside orthopedic surgeons and sports medicine physicians, and along the way has rehabilitated a wide range of patients including professional and collegiate athletes, industrial work force and geriatric populations. He specializes in the area of men’s health and is avid on breaking the stigma associated with their hesitance to access healthcare. After developing a pelvic floor condition himself and no other local male to entrust treatment, he pursued specialization to be a male provider option for males in the Central Illinois area. He is one of just a few male therapists in Illinois that specialize in treating men’s health conditions.
Prior to opening MOOSE Physical Therapy, his work history includes having managed a therapy department for an outpatient pain clinic, satellite-site supervisor, staff therapist, and lead industrial specialist for the Central Illinois market. He is active with community outreach of men’s health issues. When not practicing physical therapy, he enjoys spending time with his wife and two children, being active with his church family, watching sports, traveling, and exercising. He is currently accepting new patients - men, women and children.

Jonathan Hamm PT, DPT, OCS

Dr. Jonathan Hamm is one of the founding members of MOOSE Physical Therapy, PLLC. It has been a dream of his over the past few years to be a private practice physical therapist owner. However, knowing how busy his work and life schedules were, he didn’t really know if it would have been a reality if God had not intervened and clearly brought about a series of events that resulted in meeting and befriending Jeremy, whom was already well into the process of starting this clinic. He is extremely grateful to Jeremy for his open-mindedness about bringing on a partner. He is also very thankful to his wife, Andrea, for her unconditional love and support as they walk through this journey together. His greatest joy in life is walking with his Lord and Savior, Jesus Christ, who has brought about the events of starting this clinic and using it as a ministry for people to know Him and be filled with joy in Him. A few years ago, God spoke to his wife and told her, at a very critical time, “Just Trust” Him! Since that time, that has been their family’s mantra and they are excited to be part of this business with that mindset and with the excitement to see people healed through MOOSE Physical Therapy.
Jonathan believes that recent insurance reimbursement changes in physical therapy have unfortunately brought about a shift from personalized, one-on-one care to a less personal model - except now at MOOSE Physical Therapy, you will have that personalized care. He is committed to giving you the quality evaluation and individualized treatment plan you deserve in a caring environment where you are treated like family. He would be pleased to be ‘your’ physical therapist and looks forward to meeting you soon!
He became a physical therapist to have a direct impact on peoples’ physical health and through his years of practicing, this mission has not changed. He has enhanced his skill set with advanced training in office ergonomics evaluation, treating vertigo and vestibular disorders, in addition to the following certifications:
- Orthopedic Clinical Specialist (OCS), 2016
- Functional Dry Needling Specialist, 2019
His work history includes managing an outpatient physical therapy clinic for several years and a staff therapist position at a local rehabilitation facility. He is blessed to be married to his high school sweetheart and together have 3 amazing boys. He enjoys building forts, playing nerf gun wars, and bike riding with his family. He also enjoys watching movies, coaching, and doing anything outdoors. He said, “a unique fact about me is I once went tandem skydiving. The free fall was amazing!” A verse that has been inspirational to him to begin this business is Psalm 37:5 - “Commit your way to the Lord; trust in Him, and He will act.” He is currently accepting new patients.
